Company understands that health information about you and your health is personal. If you are under 18, please do not use the Services without parental consent. If we learn we have collected such information without consent, we will take steps to delete it. - Account Information. When you register, we collect personally identifiable and (where applicable) billing information.
- Device Information. IP address, geolocation, device identifiers, browser type and language, OS, referring/exit URLs, pages viewed, time spent, error logs, and similar usage data.
- Location Information. General (IP, ZIP) and more specific (GPS-based) location, used to customize the Services. You can disable location-tracking on your device.
- Cookies and Other Electronic Technologies. Cookies, web beacons, platform analytics, and mobile device identifiers used to improve and tailor the Services and for analytics and fraud prevention.

Most browsers accept cookies by default. You can configure your browser to remove or reject them, but the Services may not function properly.
10.3. Opting Out of Tailored Advertising
Visit the Network Advertising Initiative, the Digital Advertising Alliance, or TRUSTe's Advertising Choices page to opt out of interest-based advertising from participating companies. Opting out does not stop all data collection or non-targeted ads.
10.4. "Do Not Track" Signals
We do not currently respond to browser-initiated Do Not Track signals, as industry standards are still evolving.

This Notice is in effect from the date noted above until Company replaces it. Company reserves the right to change the terms of this Notice at any time, in compliance with applicable law. Changes apply to all PHI Company maintains. Updated notices will be posted on the Platform and made available upon request.
12.1. Uses and Disclosures of PHI
- Treatment, Payment, and Healthcare Operations. For example, disclosing PHI to a physician for treatment, to insurers for payment, or to third-party service providers under appropriate agreements.
- Authorization. With your written authorization, which you may revoke at any time.
- As Required by Law.
- Special Circumstances — including public health activities, workers' compensation, health oversight, judicial/administrative proceedings, law enforcement, decedents, organ procurement, research, threats to health or safety, and specialized government functions.
12.2. Your Rights Regarding Your PHI
- Confidential Communication. Request communications by alternate means or location.
- Restrictions. Request limits on certain uses and disclosures (Company is not required to comply, but is bound if it agrees).
- Inspection and Copies. Request to inspect and copy your PHI (reasonable fees may apply).
- Amendment. Request corrections to PHI you believe is inaccurate or incomplete.
- Accounting of Disclosures. Receive a list of certain disclosures made within the prior six years.
- Breach Notification. Be notified of breaches of unsecured PHI.
